GRADE SCHOOL

 


 

 


Undergraduate Programs


  1. Inquiry on requirements may be done at the Student Personnel Services Office/Dean’s Office.

  2. Secure Admission Form from the Student Personnel Services Office.

  3. After accomplishing the form, pay the entrance examination fee at the College Accounting Office.

  4. Go back to the Student Personnel Services Office for the schedule of entrance examination.

  5. After taking the entrance examination and undergoing interview, the student is endorsed to the college where he/she qualifies.

  6. Submit requirements to respective college if freshmen. If transferee, have the grades evaluated first by the respective Dean.

  7. A pre-enrollment form shall be released to new students whose enrollment has been approved.



  8. Present pre-enrollment form to the College Office Assistant to get a Certificate of Matriculation (COM).

  9. Student gets his/her schedule and accomplishes COM form.

  10. Consult a faculty adviser in the respective college regarding the schedule for approval and signature of the dean.

  11. Proceed to the College Accounting Office for assessment and payment.

  12. If new student, get a student number at the Registrar’s Office.

  13. Enlist name in the master list of the respective College for the release of class cards.

 

 

High School Admission and Enrolment


 

  1. New students and transferees shall undergo an entrance exam first to gauge the level of competency of the students. Moreover, interview is set by the Guidance Office with the transferees.


  2. Incoming first year students, seeking admission to the school, shall present the following requirements to the High School Registrar for assessment:

    1. Form 138 – Card

    2. Certification of Good Moral Character

    3. Photocopy of Birth Certificate

    4. 3 pieces of 1 x 1 picture

    5. Entrance Examination Result Slip


  3. Transferees, on the other hand, shall be given a recommendation form to be filled-out by the principal, guidance counselor, or adviser. It shall then be presented to the Guidance office for further evaluation along with the following requirements:

    1. Form 138 – Card

    2. Certification of Good Moral Character

    3. Entrance Examination Result Slip


    After evaluating all the requirements, the student shall then be allowed to enroll and instructed to proceed to the Registrar’s office to comply with other required documents such as:

    1. Photocopy of Birth Certificate

    2. 3 pieces of 1 x 1 picture


  4. For returning students seeking admission to the school shall present the follow:

    1. Form 138 – card

    2. Invitation Letter duly signed by the Director/Principal


  5. The students shall be asked to fill-out the Registration Form by the High School Registrar. The Registrar then shall identify on the form the corresponding discount to be deducted from the tuition fee by the assessing clerk in the Accounting Office.


  6. The students shall pay the fees to the Cashier in the Accounting Office.

 

 

Grade School Admission and Enrolment


 

  1. New students and transferees shall undergo an entrance exam first to gauge the level of competency of the students. Moreover, interview is set by the Guidance Office with the transferees.


  2. Transferees and incoming nursery and Grade 1 students seeking admission to the school, shall present the following requirements to the Grade School Registrar for assessment:

    1. a. Form 138 – Card

    2. b. Photocopy of Birth Certificate

    3. c. Certification of Good Moral Character

    4. d. 2 copies of 2 x 2 picture


  3. For returning students seeking admission to the school shall, they present the following

    1. Form 138 – Card

    2. Invitation Letter duly signed by the Director/Principal


  4. The students shall be asked to fill-up the Registration Form by the Grade School Registrar. The Registrar then shall identify on the form the corresponding fees to be paid by the students in the Cashier’s office.


  5. The students shall pay the fees to the Cashier in the Accounting Office.
